Who We Serve
We welcome adults who may need assistance with daily living in a supportive residential setting. Our home may be appropriate for individuals who require:
- Assistance with activities of daily living (bathing, dressing, grooming)
- Medication management

Each potential resident is carefully assessed to ensure our home can meet their care needs.
Admission Process
1. Initial Inquiry – Families, case managers, or healthcare professionals may contact us to discuss care needs and availability.
2. Assessment – We review the individual’s care needs, medical history, and support requirements to ensure we can provide appropriate care.
3. Tour & Meeting – We encourage families to visit our home, meet our staff, and see the living environment.
4. Care Planning – If the home is a good fit, we work with the resident and their support team to develop an individualized care plan.
5. Move-In – We coordinate a smooth transition and welcome the resident into our home community.
Referrals
We welcome referrals from:
- Hospitals
- Social workers and case managers
- Behavioral health providers
- Families and guardians
- Community organizations
